Job Summary: Responsible for Psychosical and Discharge care plans, with Physician and other interdisciplinary team members, in the provision of quality services to the residents and their families.

Job Responsibilities and Duties:

1. Through interaction with resident and families, identifies and assesses medically related psychosocial needs of residents and initiates services/treatment plan to meet their needs.

2.   Confers with other interdisciplinary team members in order to assess and meet resident's needs.

3.   Interprets psychosocial needs of residents to families and staff.

4.   Interacts with residents to help them adjust to their medical, social, spiritual, and/or emotional needs and to the nursing home setting.

5.   Informs and counsels families to keep them involved and adjusted to the placement of the resident in the nursing home.

6. Performs individual and/or group counseling/case work/therapy including but not limited to:

            1) adjustment of the resident.

            2) rehabilitation of the resident.

            3) grieving process of the resident and/or family.

7.  Monitors resident progress of physical, mental, and psychosocial functioning.

8.  Document pertinent psychosocial information regarding residents including but not limited to:

     a.  Social Service components of the MDS and CAAs.

     b.  Social Service history and assessment.

     c.  Prepare and update social service component of the plan of care.

     d.  Quarterly progress summaries including Social Service component of the MDS review.

     e.  Social service annual assessments and social history updates as necessary.

     f. Social service discharge plan and review.

9. Assists with other members of the interdisciplinary team in developing alternative therapeutic strategies with residents on psychotropic drugs to reduce drug usage.

10. Coordinates initial and quarterly staffings for residents.  Invites resident and or family/legal guardian to the staffings.

11. Communicates with other members of the interdisciplinary team to ensure the appropriate flow of information concerning resident's current status and/or any significant change in resident's condition.

12. Conducts meetings with resident's families to discuss resident's status and/or needs, when necessary.

Job Standard:

Education:  A Bachelor's degree in Social Work from an accredited school.

Experience:  0 - 1 year experience in social work within a nursing home environment.

Certification: With State of Wisconsin.  Additional CEU credits per State mandate.
