Job Summary: Assesses and provides nursing care services to residents to meet spiritual, physical, emotional, and social needs consistent with the plan of care, LindenGrove Standards of  Nursing Practice and all applicable regulations/codes/policies/procedures.  Responds in emergency situations to assure resident safety and security.  Responsible for supervision of unit staff.

Job Responsibilities and Duties:

1.      Coordinates team members in providing quality care on assigned unit.

2.      Receives verbal report and communicates all pertinent resident care information to the nursing team staff.

3.      Assesses residents to ensure they are receiving appropriate and thorough cares.

4.      Administers and documents medication/treatments, etc. within parameters of RN licensure and according to the resident care plans.  Reports changes to RNS/RNM in large. 

5.      Receives and transcribes Physician orders.

6.      Reports significant changes in residents' condition immediately to the supervisor and notifies families/physicians as directed.

7.      Maintains timely, accurate written charts/documents/records as required by law/regulations/LindenGroves' policies and procedures.  Charting must be informative, descriptive of nursing cares provided, resident's response to cares and a notation when families/physicians are notified of significant resident changes.

8.      Reviews and revises care plans as changes occur to reflect the condition of each resident.

9.      Monitors residents' rooms to assure a clean, orderly, comfortable environment.  Makes adjustments based on individual needs.

10.  Appropriately identifies problems, develops approaches and goals of  residents' care plans and communicates these to unit staff and interdisciplinary team.

11.   Writes timely nurse summaries that reflect residents' conditions, cares being given, restorative/rehabilitation interventions, etc.

12.  Assists with special nursing department projects and programs as assigned.

Job Standards:

Must be a graduate of an accredited school and currently licensed to practice as an RN in the State of Wisconsin or eligible for licensure.

Recent experience with geriatric/restorative nursing preferred. Must have the ability to understand and comply with geriatric and restorative nursing principles and knowledge of State and Federal regulations/guidelines affecting long term care.

Must be honest, dependable and exhibit a caring professional manner and promote the philosophy of LindenGrove.

Must demonstrate comprehension and current knowledge of basic nursing theory and practice.

Follows all safety, security, infection control (including Universal Standards) and hazardous materials policies and procedures
