Job Summary: Provides prescribed medical treatment, medication and personal care services to residents to meet spiritual, physical, emotional and social needs consistent with the philosophy and objectives of LindenGrove and its nursing department within the scope of gerontological nursing as well as applicable regulations, codes, policies and procedures.  Responds in emergency situations to assure resident safety and security.

Job Responsibilities and Duties:

1.      Receives verbal report and communicates all pertinent resident care information to the nursing assistants on assigned unit/area.

2.      Observes assigned residents to assure CNA's are providing appropriate and thorough cares.

3.      Administers and documents medication/treatments or procedures, etc. within parameters of LPN licensure and according to the care plans or as directed by RNS/RNM.  Reports changes to RNS or RNM in charge.

4.      Receives and transcribes Physician orders.

5.      Observes and reports significant changes in resident's condition immediately to the supervisor and notifies resident, family, physician as directed.

6.      Maintains timely, accurate and detailed documentation as required by law/regulations and LindenGrove's policies and procedures.

7.      Writes timely nursing summaries that reflect resident's condition, cares being given, restorative/rehabilitation interventions, etc. per facility policy.

8.      Assist with review and revision of care plans as changes occur to reflect the condition of each resident under the direction of the RN.

9.      Monitors residents' rooms to assure a safe, clean, orderly and comfortable environment.  Makes modifications based on individual needs.

10.  Appropriately communicates problems, approaches and goals on resident's care plan to the nursing staff.

11.  Assists the administrative staff in evaluating, reviewing, and investigating family, resident or staff complaints/concerns as requested by the director or assistant director of nursing.

12.   Reports observed issues/problems with nursing assistants' job performance to manager or supervisor.  Submits documentation regarding incidents. Participates in counseling or disciplining conferences as requested.  Assists with the performance evaluations of the CNA.

13.   Participates in resident admission, ongoing resident care or family care conferences and discharge planning as requested.

Job Standards:

Must be graduate from an accredited school and currently licensed to practice as an LPN or RN in the State of Wisconsin or a GPN eligible for licensure.

Must have the ability to understand and comply with geriatric and restorative nursing principles and knowledge of State and Federal regulations/guidelines (OBRA) affecting long-term care.

Must be honest, dependable and exhibit a caring, professional manner.

Must demonstrate comprehension and current knowledge of basic nursing theory and practice.

Openings:

Part Time (0.8 FTE) - Day Shift Available (6:30am - 2:30pm)

Part Time (0.2 FTE) - NOC Shift Available (10:30pm - 6:30am)
